All insurance quotes and products are offered through Extend Insurance Services LLC., licensed agency and/or an officer of Extend Health. Collectively, the company and/or its officers are licensed in good standing in the following states and the District of Columbia.

Premium rates are current and accurate as provided to Extend Health by contracted companies. The premium rates shown are for quoting purposes only, are not an offer of coverage and are not guaranteed. Final premium rates will be determined by the insurance company and are based on individual circumstances. As a result of underwriting, final premium rates may differ from quoted premium rates.

Existing Insurance Coverage

DO NOT CANCEL ANY EXISTING HEALTH INSURANCE UNTIL YOU RECEIVE WRITTEN CONFIRMATION FROM THE INSURANCE COMPANY TO WHICH YOU ARE APPLYING THAT YOUR NEW POLICY IS IN EFFECT.
