At Extend Health, Inc (“Extend,” “we,” or “us”), we are committed to ensuring the privacy of our users and visitors. This privacy policy explains Extend’s privacy practices for its web site (including www.extendhealth.com), and other internet services owned or operated by Extend (the "Extend Service") for users of our services. By using the Extend Service, you consent to the terms of this privacy policy. For the purposes of clarity this Privacy Policy governs your use of the online services of Extend, however your use and coverage under any insurance services offered or sold by Extend and your Benefits (defined below) shall be subject to the terms and conditions of that policy.

We value your relationship with us, so we want you to understand our privacy objectives for our web site, and how and why we collect, use and disclose the personal information about you on our web sites. If you are a customer of Extend Health, you have also received the GLBA Notice of Privacy Practices that describes Extend Health's use and disclosure of your "non-public personal financial and health information" under the Gramm-Leach-Bliley Act ("GLBA"). If you are not yet a customer, but are applying for an insurance product through this web site, any information you supply to us and our business associates and counter parties is protected as stated in that GLBA Notice.


What is Personal Information?

Personal information is information that specifically identifies an individual, such as a user name, address, telephone number, e-mail address, age, social security number, health or medical history credit card or other account number, and information about that individual’s activities when directly linked to that person. Personal information also includes demographic information such as gender, geographic area and preferences when such information is linked to other personal information that identifies you.


Information We Collect

In order for us to provide the best possible products and services, we will ask you to provide us with personal information. Your personal information is used to identify you and maintain the security and privacy of your Benefits. We do not collect any personal information about you at our web sites unless you choose to provide it to us voluntarily.


Registering with Us


We begin collecting personal information from you when you register with us. You may register with us through our website by entering your email address. Under our current process, you must create an account before requesting an application for a health insurance or other product offered on our website. You may also register with us through our Sales Support Center by providing certain personal or contact information (e.g., email address, phone number, gender, date of birth) to one of our customer service representatives. We collect personal information in the normal course of business in order to process your insurance application and in order to serve you better.


Applying for Health Insurance and Other Products


If you apply for a health insurance or other product through our website, we may ask you to provide us with personal information and/or health information relating to you and any family member who will be included on your application. This information will be used by your chosen insurance company, agent or HSA trustee or administrator to process your application. Additionally, we may ask you to provide us with credit card or bank information, which will be used by your chosen insurance company or HSA Trustee or Administrator to process your application or collect any fees associated with your application or insurance premiums upon approval of your application.


Surveys


You may have the opportunity to provide information to us through online surveys and forms. We use this information to better understand your health insurance needs and to provide quality products, services and other opportunities. We plan to use any information we obtain from you to help us continually improve the content, functionality and overall usefulness of our web site.

Insurance Companies and authorized HSA Trustees or Administrators


If you submit an application for an insurance product offered by us, then we will disclose your personal information to your chosen insurance company to process your application. If you complete an enrollment form for a health savings account ("HSA") offered through an authorized HSA trustee or administrator with whom we have associated, then we may disclose your personal information to that trustee or administrator in order to enroll you in an HSA. These partners are only allowed to use your personal information to process your requested quote, application or enrollment form and are contractually obligated to maintain strict confidentiality and security with respect to your personal information.


Service Providers


We may disclose your personal information to other companies that help us process or service your insurance application or correspond with you. For example, we may provide your personal information to a service provider to verify the validity or credit limit of your credit card. The companies we hire to process or service your insurance application or correspond with you are not allowed to use your personal information for their own purposes and are contractually obligated to maintain strict confidentiality and security with respect to your personal information.


Providing you with a quote or processing your application


We may use your personal information to get in touch with you when necessary to process your application. We or contracted third parties may use your personal information to provide you with a quote. For example, emails will be sent to you throughout the application process to inform you of the status of your application and to seek additional information that is requested as part of the application.
